with patients and families for care or services after
hospitalization.
At Hendricks
Community Hospital, discharge planning is available
through Hospital Social Services. The Social Worker, in
consultation with your physician and other members of
the healthcare team, can assist you in planning for
post-hospital care or services.

Community Hospital at no cost.

decisions about continuing care.
- Coordinate the referral process to home care or
alternative living facilities which meet your needs.
