Discharge Planning for Patient and Families

What is it?

Discharge Planning is working together with patients and families for care or services after hospitalization.

Where can I get it?

At Hendricks Community Hospital, discharge planning is available through Hospital Social Services. The Social Worker, in consultation with your physician and other members of the healthcare team, can assist you in planning for post-hospital care or services.

How much does it cost?

Discharge Planning is available to all patients at Hendricks Community Hospital at no cost.

Hendricks Community Hospital's Social Worker can assist you with the transition from one environment to another:

  1. In communication with the healthcare team, together we can develop a post-hospital plan that meets your needs. 
  2. Provide information about available community resources. 
  3. Provide counseling to patients and families who may need help in making appropriate and realistic decisions about continuing care. 
  4. Coordinate the referral process to home care or alternative living facilities which meet your needs.
